RN Health Coach - Remote

Overview

As an RN Health Coach, you will work one-on-one with SoonerCare members to provide education, support, and self-management tools aimed at improving their health.

In Short

• Work directly with SoonerCare members to enhance health outcomes.
• Assist primary care practices in building quality-driven care delivery processes.
• Evaluate and coach patients with chronic medical conditions.
• Oversee data capture and analysis to identify care delivery gaps.
• Implement and teach quality improvement methods.
• Improve practices' ability to manage patient populations through data analysis.
• Provide psychosocial assessments and crisis intervention.
• Utilize brief solution-focused therapy for member counseling.
• Guide members towards overall wellness.
• Must reside in Oklahoma.

Requirements

• Registered Nurse (RN) license required.
• Experience in health coaching or chronic care management.
• Strong communication and interpersonal skills.
• Ability to analyze data and identify improvement areas.
• Knowledge of quality improvement methods.
• Experience in providing psychosocial assessments.
• Skills in crisis intervention and counseling.
• Ability to work independently and as part of a team.
• Commitment to improving patient health outcomes.
• Must reside in Oklahoma.
