Intake Assessor - LPN - Remote

Overview

This position provides front line intake clinical screening to members who need skilled nurse assessment services, performing assessments virtually and documenting member outreach activities.

In Short

• Performs virtual assessments for individuals with disabilities.
• Documents assessment results and negotiates service plans as needed.
• Provides ongoing education and communicates with healthcare providers.
• Assists with scheduling and conducting express assessments under RN supervision.
• May require collaboration with healthcare providers for treatment plans.
• Responsible for miscellaneous duties as assigned.
• Must have a nursing degree or equivalent training/experience.
• 1-2 years of clinical experience required.
• Bilingual English/Spanish is a plus.
• Able to work flexible hours including weekends and holidays.

Requirements

• Two or four-year nursing degree and/or equivalent training and/or experience.
• Minimum of two years of experience working with persons with disabilities.
• Current and valid LPN/RN licensure in Colorado.
• Case management experience preferred.
• LPNs must perform services within their scope of licensure.
• Able to work a flexible schedule based on business needs.
