Temporary Intake Assessor - LPN - Remote

Overview

This position provides front line intake clinical screening to members who need skilled nurse assessment services, including virtual assessments and collaboration with healthcare providers.

In Short

• Performs virtual assessments of persons with disabilities.
• Documents assessment results and negotiates service plans.
• Provides ongoing education to healthcare providers.
• Assists with scheduling and conducting express assessments.

• Requires LPN/RN licensure in Colorado.
• 1-2 years of clinical experience required.
• Experience with persons with disabilities preferred.
• Bilingual English/Spanish is a plus.
• Able to work flexible schedules, including weekends and holidays.

Requirements

• Two or four-year nursing degree or equivalent training.
• Minimum of two years of experience in clinical settings.
• Current and valid LPN/RN licensure in Colorado.
• Experience in case management preferred.
• Ability to work a flexible schedule based on business needs.
• Must perform services within the scope of licensure.
• Bilingual English/Spanish is beneficial.
