In Short

• Provide integrated support to patients and physicians.
• Develop personalized relationships to improve medication adherence.
• Review and interpret patient care notes for clinical data.
• Monitor ongoing patient adherence across various disease categories.
• Evaluate EMR for medication adherence and optimize patient outcomes.
• Conduct medication therapy management (MTM).
• Supervise communication of support personnel compliant with HIPAA.
• Participate in education programs and training.
• Assist with research projects and peer reviewing articles.
• Actively participate in process improvement initiatives.

Requirements

• Doctor of Pharmacy (PharmD) or equivalent advanced degree.
• 1+ years' experience preferred but not required.
• Strong verbal and written communication skills.
• Deep knowledge of specialty medication pharmacology.
• Ability to manage processes and exhibit strong technical skills.
• Proficiency in Microsoft Word, Excel, and PowerPoint.
• Knowledge of complex/chronic disease states and therapies.
• Ability to motivate and coach patients.
• Strong attention to detail and quality focus.
• Must comply with local, state, and federal drug laws.
