Overview

Aledade is seeking a Director of Community Health Center Performance to develop and maintain data systems for tracking contract performance and supporting high-quality care at Community Health Centers (CHCs).

In Short

• Develop and manage CHC data dashboards.
• Support performance management of CHC partners.
• Lead development of new programs for CHC performance improvement.
• Collaborate with stakeholders to track performance metrics.
• Participate in Collaborative Learning Networks.
• Represent Aledade at CHC summits and events.
• Work closely with internal and external partners.
• Comfortable speaking about CHC performance.

• Periodic travel for internal meetings and CHC events.

Requirements

• Bachelor’s degree and/or 10+ years of healthcare experience.
• 2+ years working with Federally Qualified Health Centers.
• 5+ years in value-based care.
• Experience with population health analytics and data visualization.
• Able to thrive in a fast-paced environment.
• Strong program management skills.
• Excellent communication skills.
