The Patient Outreach Specialist at Zócalo Health will act as the first point of contact for new members, providing outreach services and support to enhance their healthcare experience.

Act as the first point of contact for new Zócalo Health members.
Complete consent procedures for eligible members.
Provide outreach services through phone, SMS, email, and digital platforms.
Make appointment reminder calls to patients.
Triage incoming calls and assist members.
Collaborate with the team to meet targets and metrics.
Build trust and rapport with members.
Help patients troubleshoot technical issues.
Use CRM tools to track data and communicate.
Work a full-time schedule, Monday to Friday, 9 am to 5 pm.
Requirements
Fluent in Spanish and English (verbal and written).
Experience in a high volume call center or customer service role.
Ability to work in a fast-paced environment.
Strong attention to detail and organizational skills.
