Community Health Worker/Engagement Specialist - Remote

Overview

Zócalo Health is seeking a Community Health Worker/Engagement Specialist to provide culturally aligned and accessible care to the Latino community, improving patient engagement and access to care.

In Short

• Work from home with some community engagement.
• Full-time position with a focus on patient outreach.
• Engage with patients to educate them on services and navigate health needs.
• Organize and attend community events.
• Assist with enrollment in health programs.
• Document patient interactions across multiple systems.
• Provide culturally and linguistically appropriate health education.
• Collaborate with a multidisciplinary care team.
• Assess social determinants of health needs.
• Support individualized goal setting using motivational interviewing.

Requirements

• Fluency in English and Spanish.
• High school diploma or GED.
• Community Health Worker certification or relevant experience.
• 1-3 years of healthcare experience.
• 2-5 years of community work or advocacy.
• Familiarity with Google Workspace and EHR documentation.
• Training in motivational interviewing preferred.
• Comfortable with multiple computer applications.
• Strong ties to the local Latino community preferred.
• Willingness to travel for community events.
