The Community Health Worker Team Manager at Zócalo Health is responsible for overseeing and supporting community health teams, ensuring excellence in care delivery to members.

Requirements

• Bachelor’s Degree or equivalent experience in social services.
• 3+ years of experience in a healthcare setting.
• 2+ years as a supervisor to frontline staff.
• Experience with Medicaid/Medicare.
• Fluent in English and Spanish.
• Strong communication and interpersonal skills.
• Proficient in computer skills.
• Ability to maintain HIPAA compliance.
• Passion for community service.
• Proactive learner comfortable in various environments.
