In Short

• Engage with prospective clients and record interactions in the CRM system.
• Convey client preferences to the care team for personalized care planning.
• Assist clients in completing psychosocial assessments and developing care plans.
• Review medical charts to document client needs and recommend interventions.
• Apply evidence-based practices in client interactions.
• Participate in weekly Team Operations Meetings to coordinate care delivery.

Requirements

• Active California Licensed Vocational Nurse (LVN) license, Bachelor's of Social Work, or Medical Assistant certificate.
• 3+ years of experience in healthcare, ideally with vulnerable populations.
• Excellent oral and written communication skills.
• Positive interpersonal skills for effective client engagement.
• Comfortable with technology, including video conferencing and electronic health records.
• Prior experience in a remote role is important.
• Bilingual (English/Spanish) preferred.
