Overview

The Case Coordinator provides comprehensive assessments and develops care plans to ensure a high-quality client experience while managing resources and budgets.

In Short

  • Permanent, full-time position in the SMILE Program.
  • Hybrid remote work with community visits 2-3 times a week.
  • Responsible for client assessments and care plan development.
  • Facilitates access to community and health resources.
  • Requires travel to client homes and possibly to the Trenton office.
  • Engages with clients to monitor progress and adjust care plans.
  • Maintains confidentiality and adheres to privacy policies.
  • Collaborates with community partners and stakeholders.
  • Requires a degree/diploma in Health or Social Services.
  • 3 years of experience in home/community care or social services.

Requirements

  • University Degree or College Diploma in Health or Social Services.
  • Minimum 3 years related experience in case management.
  • Experience with standardized assessment tools.
  • Strong communication and relationship management skills.
  • Proficiency in Microsoft Office and analytical skills.
  • Ability to manage multiple tasks and work independently.
  • Knowledge of community resources and services.
  • Strong attention to detail and organizational skills.
  • Must have access to a personal vehicle.
  • Current Criminal Records Check required.
