Overview

The Palliative Care Nurse Navigator plays a crucial role in supporting patients with serious illnesses, ensuring they receive comprehensive care and guidance throughout their healthcare journey.

In Short

• Conduct conversations about goals and values with patients.
• Perform comprehensive symptom assessments.
• Support members longitudinally and facilitate end-of-life discussions.
• Engage with members via phone, email, and video communication.
• Collaborate with Nurse leaders and Medical Directors.
• Establish trusting relationships with members and their care network.
• Participate in case conferences to monitor member progress.
• Coordinate care and provide clinical support as needed.
• Be available for urgent clinical escalations.
• Perform other related duties as assigned.

Requirements

• Bachelor of Science Degree in Nursing required.
• Compact unrestricted registered nurse (RN) license required.
• Preferred licensure in New York, Illinois, and/or California.
• At least 5 years of nursing experience required.
• 3 years in outpatient palliative care or home hospice preferred.
• Certifications in hospice and palliative care are a plus.
• Strong communication and organizational skills.
• Ability to work in fast-paced environments.
• Proactive and adaptable to change.
• Member-centric approach to care.
