Overview

The Oncology Case Manager will play a crucial role in transforming the cancer care experience for patients and their caregivers, providing support and guidance throughout the oncology care continuum.

In Short

  • Conduct clinical assessments and monitor health changes.
  • Coordinate care and transitions for cancer patients.
  • Educate members and caregivers about diagnoses and treatments.
  • Support culturally competent care.
  • Provide telephonic assessments and follow-ups.
  • Build relationships with members, payers, and providers.
  • Assist with care management post-admissions.
  • Utilize technology for communication and documentation.
  • Work remotely or hybrid from Nashville office.
  • Full-time position with a pay rate of $45.67/hour.

Requirements

  • Bachelor of Science Degree in Nursing (BSN).
  • Unrestricted Registered Nurse (RN) license.
  • 5 years of nursing experience, with 3 years in oncology or case management.
  • Certified Case Manager (CCM) required.
  • Strong organizational skills and ability to manage multiple tasks.
  • Excellent communication and listening skills.
  • Ability to adapt to a fast-paced, changing environment.
  • Comfort with using new technologies.
  • Ability to identify and prioritize member needs.
  • Willingness to work up to 2 holidays per year.
