Outreach Coordinator - Remote

Overview

BKG is seeking a Member Outreach specialist who can provide resources who are trusted members of the communities served and/or have an unusually close understanding of the communities to facilitate access to health care services, improve the quality and cultural competency of those services, and improve member health outcomes.

In Short

• Full-time position as an Outreach Coordinator.
• Work remotely with local travel required.
• Pay rate of $20.00/hr plus mileage reimbursement.
• Monday to Friday schedule, 8:00 AM to 5:00 PM.
• Focus on increasing health literacy and improving member health outcomes.
• Engage members in care coordination and case management.
• Facilitate health services and monitor utilization.
• Utilize a whole health approach in interactions.
• Pivot priorities based on HEDIS performance.
• Work with Case Management to enhance member engagement.

Requirements

• Community Outreach Experience preferred.
• CHW Certification and/or CNA/HHA preferred.
• Covid Vaccinated.
• High School Diploma/GED required.
• Driver’s License required.
• Home Visits (local) necessary.
