Overview

We are building the tools and designing an approach to enable care to reach the patients who can benefit most.

In Short

• Lead outreach & care coordinators in the market.
• Supervise day-to-day activities of care coordinators.
• Participate in patient outreach and care coordination tasks.
• Provide coaching and mentoring to team members.
• Collaborate with multidisciplinary care teams.
• Review performance data to meet goals.
• Support hiring and training of care coordinators.
• Build partnerships with primary care providers.
• Engage with community events to enhance partnerships.
• Foster an inclusive team environment.

Requirements

• Commitment to improving healthcare for underserved populations.
• Experience supervising care coordinators or similar roles.
• Understanding of Medicaid and healthcare access barriers.
• Strong analytical skills with data monitoring experience.
• Effective communication in a multidisciplinary setting.
• Experience with healthcare data and Electronic Health Records.
• Ability to travel locally to support teams.
