Outreach Specialist / Care Coordinator Manager - Remote

Overview

Guided by the communities we serve, we bring support and technology-enabled care to help primary care providers keep Medicaid patients healthy. We are building the tools and designing an approach to enable care to reach the patients who can benefit most.

In Short

• Lead outreach and care coordinators in the market.
• Participate in patient outreach and care coordination tasks.
• Provide coaching and support to care coordinators.
• Collaborate with multidisciplinary teams.
• Manage performance metrics and goals.
• Support hiring, training, and deployment of care coordinators.
• Build partnerships with primary care providers.
• Engage with community events and partnerships.
• Foster an inclusive team environment.
• This role is mainly remote with some travel required.

Requirements

• Commitment to improving healthcare for underserved populations.
• Experience supervising teams in healthcare or related fields.
• Understanding of Medicaid and healthcare access barriers.
• Strong analytical skills, particularly in Excel.
• Effective communication in a multidisciplinary setting.
• Experience with healthcare data or Electronic Health Records.
• Ability to travel within the local area.
