Overview

The Specialty Care Coordinator is responsible for helping NeighborHealth Center by working collaboratively to support patient care plans and needs. Serving as an advocate for patients, specifically focusing on behavioral health and prenatal care, the Specialty Care Coordinator will be part of the care management team and work in tandem with healthcare providers and community-based organizations to improve outcomes for patients they serve.

In Short

  • Coordinate behavioral health patient appointments and follow-ups.
  • Assist with the coordination of care for prenatal patients.
  • Serve as a liaison among patients, community services, and care team members.
  • Maintain appropriate documentation in the EMR.
  • Support general care coordination duties as required.
  • Provide educational information to patients and families.
  • Attend departmental meetings and training events as needed.
  • Build relationships with community resources/providers.
  • Participate in quality improvement initiatives.
  • Other duties as assigned.

Requirements

  • High School education or equivalent.
  • Excellent verbal and written communication skills.
  • Ability to work in a team environment.
  • Proficient in MS Office and computer skills.
  • Current NC driver's license and auto insurance.
  • Ability to maintain confidentiality.
  • Experience in patient advocacy.
  • Ability to work a Monday to Friday schedule with flexibility.
  • Skills in organizational and critical thinking.
  • Must have required vaccinations.
