Requirements

• 3+ years experience in health-related field.
• Bachelor's Degree or equivalent experience.
• Preferred certifications: CRC, CDMS, CRRN, COHN, or CCM.
• Experience with Medicare and Medicaid.
• Knowledge of assessment, screenings, and care planning.
• Bilingual (English/Spanish or English/Creole) preferred.
• Managed care experience is a plus.
• Experience working with geriatric and behavioral health populations.
