The PACE Outreach Coordinator is responsible for developing and implementing outreach strategies to increase referrals for the PACE program, focusing on building relationships within the community.

Requirements

• Bachelor’s degree in healthcare or human services preferred.
• Minimum of two years’ experience in direct sales or healthcare.
• Experience working with the frail and elderly is required.
• Food handling permit required upon hire.
• Strong attention to detail and organizational skills.
• Ability to interact positively with participants and families.
• Excellent judgment and discretion in confidential matters.
• Must be a licensed driver with a reliable vehicle.
• Compliance with health and vaccination policies is expected.
