Service Coordinator Clinician - Remote

Overview

We are seeking a dedicated and compassionate Service Coordinator (Clinical) to join our team. In this role, you will play a vital role in our care coordination process by conducting assessments, identifying member needs, and developing person-centered service plans.

In Short

• Conduct assessments to identify the needs of our members.
• Collaborate with care coordinators to ensure comprehensive support.
• Participate in team rounds with interdisciplinary teams.
• Travel to members' homes for assessments.
• Focus on clinical assessments and support for behavioral health needs.
• Work Monday to Friday from 8am to 5pm.
• Receive weekly pay and reimbursable mileage.

Requirements

• Current, unrestricted LSW, LCSW, or LMSW license required.
• Minimum of 2 years of relevant experience.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team.
• Compassionate and patient-centered approach to care.
