LTSS Service Coordinator - RN Clinician - Remote

Overview

We are seeking a dynamic and experienced LTSS Service Coordinator - RN Clinician to join our team, responsible for managing members' cases and ensuring compliance with applicable state laws.

In Short

• Manage members' cases within the scope of licensure.
• Conduct clinical assessments to evaluate members' needs.
• Coordinate care in collaboration with the health care team.
• Develop and monitor individualized care plans.
• Establish goals in collaboration with members and caregivers.
• Identify members who would benefit from alternative levels of care.
• Assist in the implementation of member care plans.
• Interface with Medical Directors and Inter-Disciplinary Teams.
• Supervise the work of non-RN clinicians.

Requirements

• Registered Nurse (RN) with a minimum of 3 years of experience.
• Current, unrestricted RN license in Indiana.
• MA/MS in Health/Nursing preferred.
• Training in case management and care coordination services.
• Strong communication and organizational skills.
• Ability to work effectively in a collaborative team environment.
