Service Coordinator Clinician - Remote

This role involves conducting assessments and developing service plans for individuals with chronic illnesses and disabilities, requiring both remote work and travel.

Requirements

• Current, unrestricted RN, LSW, LCSW, or LMHC license.
• Minimum of 2 years of experience in a similar role.
• Strong communication and interpersonal skills.
• Ability to work in a team.
