In Short

• Act as a liaison between provider network and nephrology practices.
• Facilitate transition to Value-Based Care.
• Communicate complex information clearly to stakeholders.
• Provide outstanding customer service to all stakeholders.
• Investigate and resolve issues promptly.
• Serve as primary contact for IWH Network practices.
• Assist nephrology partners with data insights.
• Ensure timely completion of deliverables.
• Maintain accurate documentation in CRM.
• Deliver training to Network Providers as needed.

Requirements

• Bachelor’s degree required; Advanced Degree desirable.
• Experience working with physicians; nephrology experience a plus.
• Excellent attention to detail and follow-through.
• Proactive approach and dedication to continuous learning.
• Excellent communication and interpersonal skills.
• Willingness to travel up to 50% of the time.
