RN Clinical Liaison/Quality Practice Advisor (Hybrid) - Remote

Overview

The Registered Nurse, Quality Practice Advisor is responsible for coordinating performance improvement activities to engage, educate, and manage activities to improve the health outcomes of Healthmap Solutions members.

In Short

• Develop partnerships with clinical providers and practice locations.
• Identify opportunities to improve health outcomes based on provider data.
• Incorporate education on Best Practice sharing for low performance areas.
• Assist in identifying areas for process improvement in workflows.
• Partner with physicians to identify members needing Care Navigation support.
• Support stakeholders in process improvement initiatives.
• Improve provider performance in health interventions and care gaps.
• Educate providers on HEDIS measures.
• Support development of quality improvement interventions.
• Document all provider meetings for consistency.

Requirements

• Bachelor’s degree required.
• Active, unrestricted RN license required.
• Basic Life Support (BLS) certification required.
• 3 years of experience in healthcare or managed care.
• Experience in claims or gap closure campaigns preferred.
• 3+ years in healthcare services, clinical operations, or quality.
• Experience building relationships with healthcare providers preferred.
• Valid driver’s license and automobile insurance required.
• Same state residency required.
