Overview

The Referral Coordinator is responsible for triaging incoming patient referrals and ensuring a smooth referral experience for providers in a fully virtual healthcare environment.

In Short

  • Collaborate with admissions and outreach teams.
  • Serve as a liaison between providers and outreach managers.
  • Manage data in multiple systems including CRM and EMR.
  • Handle confidential health information carefully.
  • Ensure accurate data input and assist with audits.
  • Potential travel for conferences and events.
  • Communicate confidently about Equip’s services.

Requirements

  • Bachelor’s degree or higher.
  • 1+ years experience in customer service or case management, preferably in healthcare.
  • Excellent interpersonal and communication skills.
  • Highly organized with strong attention to detail.
  • Comfortable working remotely with high autonomy.
  • Ability to think critically and adapt to changes.
  • Desire to work in a fast-growing health tech startup.
