Overview

This fully remote role places you at the heart of care coordination, supporting individuals on their mental health journey. You will serve as the key point of contact between clients, healthcare partners, and internal teams, ensuring timely and accurate referrals through various platforms.

• Coordinate referrals by interacting with healthcare systems, clients, and external partners.
• Facilitate documentation and track client progress throughout their mental health care episodes.
• Manage and route inbound and outbound communications via phone, email, and electronic systems.
• Process referral tasks accurately and on time using EMRs and related technologies.
• Conduct outreach to gather necessary information for connecting clients to appropriate care.
• Provide administrative support for post-session documentation and lead management.
