The Community Health Worker serves as a vital link between health/social services and the community, focusing on maternal and child health needs.

Requirements

• Must reside in the Fort Wayne area.
• Ability to complete 35 in-person community visits per week.
• Maternal/Child Health experience preferred.
• Strong interpersonal and communication skills.
• Ability to communicate with diverse backgrounds.
• Superior organization skills.
• Must work independently and effectively with others.
• High degree of computer literacy preferred.
• Ability to handle protected health information in accordance with HIPAA.
