The Community Health Worker (CHW) plays a crucial role in facilitating access to healthcare services for members, ensuring they receive the support they need through direct contact and coordination with various resources.

• High degree of computer literacy preferred.
• Understanding of HIPAA requirements.
• Bilingual candidates preferred.
• Access to reliable transportation and valid driver’s license.
