Certified Community Health Worker - Remote

Overview

The Certified Community Health Worker serves as the initial and main point of contact between the company and the member, supporting member access to care through health screenings and needs assessments.

In Short

• Responsible for coordinating follow-up care plans for members.
• Provides telephone and on-site Recovery Support.
• Makes referrals to Case Management Services.
• Assesses member compliance with medical treatment plans.
• Identifies barriers to plan compliance and coordinates resolutions.
• Assists members in accessing community resources.
• Recommends treatment plan modifications.
• Participates in cross-functional teams on projects and initiatives.
• Maintains confidentiality of sensitive information.
• Accountable for understanding role in utilization management benchmarks.

Requirements

• Ability to complete 35 community visits per week.
• Must be fully vaccinated against COVID-19 and flu.
• Strong interpersonal and communication skills.
• Ability to communicate effectively with diverse populations.
• Superior organization skills.
• Must work independently and collaboratively.
• High degree of computer literacy preferred.
• Ability to handle protected health information in accordance with HIPAA.
• Must reside within Fort Wayne, Indiana area.
• Community Health Worker certification required.
