Overview

This position serves as a liaison between members, providers, and community resources to improve health outcomes and support wellness initiatives.

In Short

  • Engage with members to provide health literacy and resource linkage.
  • Conduct outreach for preventive health and chronic condition initiatives.
  • Coordinate access to health services with community agencies.
  • Develop rapport with community centers to empower members.
  • Assist members in completing health assessments and addressing barriers.
  • Collaborate with human services providers for member identification.
  • Prepare educational materials and conduct community training.
  • Participate in innovative program development for at-risk members.
  • Represent the company at community events.
  • Other duties as assigned.

Requirements

  • High School Diploma/GED required.
  • 1 year of experience in healthcare customer service or marketing.
  • Basic medical terminology knowledge preferred.
  • Experience with targeted communities or populations preferred.
  • Strong customer service principles and problem-solving skills.
  • Ability to work as a team member and interact well with others.
  • Good written and oral communication skills.
  • Cultural sensitivity and competency in working with diverse populations.
  • Patience and empathy in member interactions.
  • Ability to handle multiple tasks simultaneously.
