Overview

The Medical Case Manager ensures that members with complex medical and psychosocial needs receive high-quality, cost-effective healthcare through assessment, planning, coordination, and advocacy.

Requirements

• Experience with Medicaid or Medicare populations.
• Licensure as a social worker or RN.
• Bilingual in English/Spanish preferred.
• Case Management Certification preferred.
• Strong communication and problem-solving skills.
• Ability to work independently.
• Knowledge of medical terminology.
• Experience with high-risk populations preferred.
