Overview

The Behavioral Health Case Manager for Transitions of Care is dedicated to proactive engagement with members admitted to Acute Inpatient Psychiatric facilities, coordinating care to address immediate behavioral health needs and overcoming barriers to discharge.

In Short

  • Coordinate care for members in psychiatric facilities.
  • Reduce readmissions through patient education and support.
  • Engage in telephonic outreach and local visits.
  • Manage a fluid caseload based on facility admissions.
  • Perform case management activities in community settings.
  • Ensure effective implementation of interventions.
  • Maintain communication between members and providers.
  • Meet licensing and certification requirements.
  • Utilize Microsoft Office for documentation and communication.
  • Support utilization management functions.

Requirements

  • Current Tennessee license in Nursing or behavioral health field.
  • 3 years of clinical behavioral health experience required.
  • 1 year of knowledge about community care resources.
  • Certified Case Manager credential preferred.
  • Proficient in Microsoft Office applications.
  • Strong decision-making and problem-solving skills.
  • Excellent oral and written communication skills.
  • Strong interpersonal and organizational skills.
  • Ability to engage diverse populations.
  • Experience with Motivational Interviewing Techniques.
