• Providing health coaching for behavior change
• Maintaining a resource directory of local and national resources
• Documenting care in electronic medical records
• Participating in regular team meetings

Requirements

• Passionate about caring for the community
• Team player with a can-do attitude
• Highly organized and comfortable multi-tasking
• Skilled at advocating for patients
• Culturally competent and enthusiastic about diverse groups
• Resident of the Boston Metro area with local resource knowledge
• Experience with medically complex, elderly, or frail populations
• Comfortable with in-person visits and telehealth
• Valid driver's license and access to an insured vehicle
• Community Health Worker Certification or higher education preferred
