• Master’s degree in Social Work or Public Health preferred.

Requirements

• Master’s Degree in Social Work, Public Health, or equivalent experience.
• Proficiency in using technology for remote support.
• Strong problem-solving skills and adaptability.
• Excellent organizational and communication skills.
• Familiarity with the Greater Chicago area and its resources.
• Community Health Worker certification preferred.
• Experience with managed care patients is a plus.
• Knowledge of Chicago Medicaid populations is beneficial.
