Clinical Pharmacist - West Coast - Remote

Overview

The Clinical Pharmacist at ConcertoCare is responsible for drug therapy management and chronic condition disease management, working collaboratively with healthcare providers to ensure patient safety and optimal medication use.

• Execute pharmacist-led disease state management.
• Maintain a patient panel for chronic conditions.

• Assist in decreasing medication cost utilization.
• Must hold an active pharmacist license in California.
• 3 years of pharmacy experience or PGY1 Residency required.
• Experience in ambulatory or managed care settings preferred.
• Bachelor's degree or PharmD required.

Requirements

• Active, unrestricted pharmacist license in California.
• Experience in multiple markets preferred.
• Strong communication skills required.
• Motivated self-starter.
• Chronic condition management experience preferred.
• Knowledge of managed care and Medicare/Medicaid is a plus.
• Open to obtaining licenses in additional states.
• 3 years of pharmacy experience or ASHP-accredited PGY1 Residency.
• Experience in clinical settings is preferred.
• Strong interpersonal skills required.
