In Short

  • Connect with patients to learn about their unique health history and current needs as it relates to cancer prevention, risk reduction, and screening.
  • Provide basic education and counseling on cancer risk reduction and prevention.
  • Develop cancer screening and risk reduction plans for patients.
  • Triage patients as necessary to other care team members at Color.
  • Navigate patients to the right external care setting.
  • Motivate patients to complete medical and non-medical action plans.
  • Build long-term, trust-based relationships with patients.
  • Schedule patients for external appointments and coordinate their care.
  • Deliver gold-standard service to patients via messaging, phone, and video.
  • Other responsibilities as needed.

Requirements

  • A background in allied health, human services or healthcare, with 3-5 years of experience supporting patients.
  • Background and extensive experience counseling and working directly with patients.
  • A friendly, empathetic problem solver.
  • Team-focused; effective collaborator within the care team.
  • Exceptional written and verbal communication skills.
  • Strong clinical skills, and high bar for experience and quality of care.
  • Excellent organizational skills with the ability to multitask.
  • Comfort with telehealth technology.
