Integrated Clinic Pharmacist - Remote

Overview

This position integrates the pharmacist as a member of the care team, responsible for managing patients’ drug therapy to ensure optimal, cost-effective care delivery and improve health outcomes.

In Short

• Integrate as a member of the care team.

• Support full physician panel regardless of health plan.
• Hybrid remote and onsite position (3-4 days remote, 1-2 days onsite).
• Must be licensed in Kentucky, South Carolina, or Tennessee.
• Participate in interdisciplinary team meetings.
• Document all activity within clinic EMR.
• Support CMS Stars quality measures.
• Occasional travel for training or meetings.
• Eligible for bonus incentive plan.

Requirements

• Active and unrestricted pharmacist license.
• Pharm.D. or advanced pharmacy degree.
• 2+ years’ experience as a practicing pharmacist.
• Certified in BLS and immunization administration.
• Excellent organization and communication skills.
• Ability to work in a team environment.
• Strong attention to detail.
• Intermediate competency with MS Office.
• Preferred: Clinical residency in ambulatory care.
• Preferred: Board certification in geriatric pharmacy.
