In Short

  • Full-time position, 40 hours a week with 8-hour shifts, Monday-Friday.
  • Required weekend coverage every 3rd weekend and every other holiday.
  • Work-from-home telephonic nurse position.
  • Support patients transitioning from post-acute services.
  • Communicate with clinical teams for timely patient follow-ups.
  • Provide education on home health, hospice, and palliative care services.
  • Requires an Associates Degree, BSN preferred, and RN license.
  • 3 years of post-acute experience required.
  • Excellent analytical and communication skills are essential.

Requirements

  • Associates Degree required; BSN preferred.
  • RN license in a compact state is required.
  • 3 years of post-acute experience.
  • Home care or hospice experience preferred.
  • Current CPR certification.
  • Good time management skills.
  • Excellent problem-solving skills.
  • Fluent in English.
