Divisional RN Care Navigator - Remote

Overview

Working within an interdisciplinary care team, the Care Navigator is responsible for proactively engaging patients identified as high-risk and implementing targeted interventions to address social needs and increase access to care.

In Short

• Conduct Transitions of Care Management for a subset of the patient population.
• Provide triage guidance and supportive consultation to team members.
• Develop care plans leveraging Geriatric best practice framework.
• Identify existing barriers to engagement with necessary resources.
• Serve as a liaison between the patient and direct care providers.
• Initiate care planning and action steps for high-risk members.
• Lead interdisciplinary team meetings when indicated.
• Document patient encounters in medical record system.
• Maintain patient confidentiality in accordance with HIPAA.
• Participate in and lead interdisciplinary review of complex patients.

Requirements

• Registered Nurse (RN license).
• Minimum of 4 years of experience in human services.
• Experience with patients with behavioral health conditions preferred.
• Bilingual in English/Spanish preferred.
• Ability to multi-task in a fast-paced environment.
• Excellent organizational skills.
• Strong interpersonal and relationship building skills.
• Advanced oral and written communication skills.
• Critical thinking and problem-solving capabilities.
• Flexibility to adjust in an evolving role.
