In Short

• Seeking an exceptional Licensed Vocational Nurse (LVN) Case Manager / Clinical Lead Care Manager (CLCM).
• Serves as the client’s primary point of contact and coordinates care with various providers.
• Manages client cases and facilitates access to care.
• Collaborates with caregivers and other providers to promote wellness and member empowerment.
• Hybrid position requiring travel throughout the Imperial County area.
• Assess member needs in various health areas.
• Oversees development of client care plans and goals.
• Advocates on behalf of clients with healthcare professionals.
• Utilizes evidence-based practices in care management.
• Maintains up-to-date patient health records.

Requirements

• Willing to work Monday-Friday 8:30am-5:00pm, both in the field and remotely.
• Current California LVN license required.
• 2+ years experience as a care manager or similar role.
• Knowledge of government and community resources related to health.
• Excellent oral and written communication skills.
• Positive interpersonal skills necessary.
• Clean driving record and valid driver's license required.
• General computer skills and knowledge of Google Workspace and MS Office.
