In Short

• Supervise interdisciplinary team members, including Registered Nurses, Social Workers, and Medical Assistants.
• Ensure adequate and efficient patient care coverage.
• Manage clinical quality initiatives.
• Support the achievement of KPIs assigned to the care delivery team.
• Collaborate with Providers and multidisciplinary professional staff to enhance patient care.
• Provide leadership in conflict resolution, mentoring, and skilled decision making.
• Direct patient care delivery as needed to support organizational goals.
• Assist in onboarding newly hired clinical staff.
• Ensure team regulatory compliance.

Requirements

• Current California RN license required.
• 5+ years in direct care delivery role as a RN or MSW.
• 2+ years of experience in palliative care, hospice care, or complex care management.
• 2+ years of people management experience.
• Adheres to the company's policies and guidelines.
• Utilizes video conferencing technology effectively.
• Demonstrates efficient resource management.
• Engages in fostering positive relationships with stakeholders.
• Maintains strict confidentiality of information.
• Strong understanding of cultural competency with the target population.

• Salary Range: $90,000 - $115,000 annually.
• California Salary Range: $100,000 - $125,000 annually.
