Remote Nurse Practitioner

Overview

The Prescribing Nurse Practitioner plays a key role in providing accessible and convenient care for our members through Telehealth, focusing on chronic cardiometabolic conditions.

In Short

• Provide direct patient care and support through treatment plans.
• Issue medication prescriptions according to treatment guidelines.
• Order and interpret laboratory tests for patients.
• Educate patients on medications and treatment expectations.
• Engage in follow-up communication with patients.
• Refer patients to specialized care when necessary.
• Maintain high-quality clinical documentation.
• Provide empathetic medical care and education.
• Reach out to members identified by remote monitoring protocols.
• Work flexible hours including evenings and weekends.

Requirements

• Active, unrestricted NP license in 15+ states.
• Availability for at least three critical outreach shifts per week.
• Experience treating CHF, COPD, Diabetes, and Obesity preferred.
• Experience in integrated clinical settings.
• Strong organizational skills and attention to detail.
• Experience with quality assurance practices preferred.
• Proficient in digital technology and EMR systems.
• Ability to work independently with accountability.
• Strong problem-solving and leadership skills.
• Aptitude for working in a rapidly changing environment.
