Remote Nurse Practitioner

Overview

The Cadence Health team seeks a remote-based Nurse Practitioner to serve as the point person to help patients navigate their remote care journey, managing treatment plans and medications in collaboration with the patient’s physician.

In Short

• Oversee monitoring of patient vitals and symptoms.
• Manage treatment plans and medications for chronic diseases.
• Respond to patient escalations identified via remote monitoring.

Requirements

• 5+ years experience treating chronic diseases.
• Active multi-state compact license.
• ANCC or ANNP certification required.
• Master’s Degree as a Nurse Practitioner.
• Experience managing CHF with GDMT.
• Stable wifi for remote work.
