Nutritionist, Population Health Case Management - Remote

Overview

The Nutritionist will support population health initiatives by providing nutrition counseling and education as part of a multidisciplinary case management team, focusing on improving health outcomes and enhancing chronic disease management.

Requirements

• Bachelor’s Degree in Food and Nutrition or related field.
• Registration by the Academy of Nutrition and Dietetics required.
• Current licensure in Maryland as a Dietitian/Nutritionist.
• 6 months of clinical experience as a Registered Dietitian.
• Specialty Fellowship Program or Certification preferred.
• Master's degree in Nutrition preferred.
• Certified Diabetes Educator (CDE) preferred.
