Requirements

• Bachelor's Degree in Social Work, Psychology, Nursing, or related field.
• At least 2 years of experience in case management or behavioral health.
• Strong understanding of Social Determinants of Health.
• Excellent communication skills.
• Comfortable with Google Suite and Microsoft Office.
• Attention to detail and ability to multitask.
