Requirements

• Passion for delivering care to patients with chronic illnesses.
• Minimum of two years' experience as a Nurse Practitioner.
• Current unrestricted NP license to practice in the state.
• Current DEA license required or eligible to obtain within 90 days.
• Current CPR or BLS certification.
• Reliable transportation and high-speed internet required.
• Preferred experience in chronic disease management.
• 5+ years’ experience in relevant nursing fields preferred.
• Experience in managing chronic diseases like CHF or diabetes.
• Ability to conduct Health Risk Assessments on high-risk patients.
