Overview

As a Palliative Care Nurse Navigator at Thyme Care, you will be a vital clinical resource for members with serious illnesses, guiding them through their healthcare journey and ensuring they receive the support they need.

In Short

• Conduct conversations about goals and values with patients.
• Perform comprehensive symptom assessments.
• Support members longitudinally through their illness.
• Facilitate end-of-life care discussions when appropriate.
• Engage with members via phone, email, and video communication.
• Collaborate with healthcare teams to ensure best practices.
• Address clinical issues and manage escalations.
• Participate in case conferences to monitor member progress.
• Foster partnerships with payers and providers.
• Maintain quality standards in documentation and operations.

Requirements

• Bachelor of Science Degree in Nursing required.
• Compact unrestricted registered nurse (RN) license.
• Preferred licensure in New York, Illinois, and/or Michigan.
• At least 5 years of nursing experience required.
• 3 years in outpatient palliative care and/or home hospice.
• Exceptional communication and organizational skills.
• Ability to handle dynamic environments and multitask.
• Certifications in hospice and palliative care preferred.
