Overview

As a Palliative Care Nurse Navigator at Thyme Care, you will play a crucial role in supporting patients with serious illnesses, guiding them through their healthcare journey with empathy and expertise.

In Short

• Conduct comprehensive symptom assessments and facilitate care discussions.
• Engage with members via phone, email, and video communication.
• Collaborate with healthcare teams to ensure quality care delivery.
• Support members and their families through serious illness and end-of-life care.
• Participate in case conferences to monitor member progress.
• Be available for urgent clinical escalations and provide consult support.
• Adhere to care team policies and documentation standards.
• Work flexible shifts, including occasional Saturdays.
• Foster partnerships with payers and providers to optimize care.
• Contribute to projects and duties as needed.

Requirements

• Bachelor of Science Degree in Nursing required.
• Compact unrestricted registered nurse (RN) license required.
• Preferred licensure in New York, Illinois, and/or California.
• At least 5 years of nursing experience, with 3 years in palliative care.
• Excellent communication and organizational skills.
• Ability to work in fast-paced environments.
• Member-centric approach and adaptability to change.
• Certifications in hospice and palliative care preferred.
