Registered Nurse Patient Care Navigator - Remote

Overview

The Registered Nurse Patient Care Navigator is a key contributor to LucyRx’s clinical programs. This role combines clinical oversight, member engagement, and navigation support to optimize the utilization of GLP-1 therapies. The nurse will empower members to access complementary lifestyle resources, overcome barriers, and achieve their health goals. Support other clinical services as required.

In Short

• Evaluate member eligibility for GLP-1 therapies based on clinical guidelines.
• Monitor member progress, ensuring treatment effectiveness in collaboration with pharmacists.
• Identify and address barriers to medication adherence to support successful outcomes.
• Conduct initial and ongoing consultations to discuss therapy goals and progress.
• Educate members on proper medication use, potential side effects, and lifestyle changes.
• Provide motivational coaching to drive engagement with approved lifestyle vendors.
• Assist members in accessing resources such as Personify Health and Vida Health.
• Document member interactions and clinical assessments in the management system.
• Collaborate with the analytics team to track outcomes and generate insights for clients.
• Support other clinical services as required.

Requirements

• Active Registered Nurse (RN) license in any US state.
• 2+ years of experience in care coordination, chronic disease management, or a related field.
• Exceptional communication and interpersonal skills.
• Familiarity with weight management or diabetes programs preferred.
• Strong analytical and problem-solving skills and detail oriented.
• Ability to adapt and perform duties efficiently whenever unexpected changes arise.
• Works well in a fast-paced environment.
• Knowledge of HIPAA regulations.
• Excellent organizational, written and verbal communication.
• Highly motivated and good interpersonal skills, self-starter.
