Overview

The Outpatient Case Manager provides remote, client-centered support across all stages of treatment, ensuring efficient delivery and documentation of wraparound services.

In Short

  • Primary point of contact for outpatient clients and families.
  • Coordinate in-person and virtual therapy appointments.
  • Document appointments and manage clinical schedules.
  • Track and report no-shows and cancellations.
  • Monitor client participation and follow up on attendance.
  • Coordinate client intakes and discharges.
  • Identify barriers to care and link to resources.
  • Participate in interdisciplinary team meetings.
  • Escalate urgent issues to Clinical Director.
  • Utilize electronic systems for tracking client activities.

Requirements

  • 2+ years of experience in behavioral health or case management.
  • Strong written and verbal communication skills.
  • Proficiency in Google Workspace and/or Microsoft Office.
  • Reliable high-speed internet for remote work.
  • Ability to obtain a Level One Fingerprint Clearance Card.
