Overview

The Community Care Team Manager role holds a supervisory scope of work while balancing a small member portfolio, which reflects the work of the Community Guides at a leadership capacity. The Manager leads by example in supporting our mission, is a champion of team leadership, and is accountable for our Care Team Program and its Community Guides.

In Short

  • Supervise Community Guides and manage a small member portfolio.
  • Lead remote Care Teams through change management.
  • Facilitate regular meetings and performance reviews.
  • Support training and development of new staff.
  • Establish trusting relationships with members.
  • Utilize various communication methods to engage with members.
  • Ensure compliance with HIPAA and confidentiality policies.
  • Participate in the Guide Interview process.
  • Provide support and supervision to Guides.
  • Contribute to community outreach efforts.

Requirements

  • Bachelor’s degree in Social Work, Human Services, or related fields.
  • Three years of program management/supervision experience.
  • Five years of direct experience with local populations.
  • Proficiencies in critical thinking and time management.
  • Knowledge of crisis planning and case management.
  • Strong relationship-building skills.
  • Ability to develop and execute outreach plans.
  • Working knowledge of Microsoft Office and video conferencing tools.
